I give my child or ward permission to attend New Creation-Camp. I have read the rules and understand that if my child behaves in an unacceptable manner, they may be dismissed from the camp at my expense. I have reviewed these rules with my child, hereafter referred to as the “student,” and the student agrees to abide by them.

I give permission for my child’s photo or video to be taken and used for RBC social media, publications, or website. If I do not wish for my child’s photo to be used, I will inform their adult leader and make sure my child identifies themselves at the registration desk at check-in.

I authorize my child or ward to participate in all New Creation-Camp activities at the location and on the dates specified, except where I have stated otherwise. I give permission for my child to be transported by adult leaders as needed during the camp, in accordance with youth protection policies.

In the event of a medical emergency where I cannot be reached, I give the Camp trip director or a designated representative permission to choose a physician and/or hospital for my child’s care. I also authorize that medical personnel may perform necessary procedures including hospitalization, tests, x-rays, anesthesia, injections, or surgery.

I release and hold harmless organisation representatives and Camp staff from any liabilities arising from medical treatment given to my child. I understand that I am fully responsible for all medical expenses incurred, and I agree to notify my insurance provider and ensure payment of these costs.
